What do you consider villains flatting range OTB if he is 23/21 over 250+ hands?

Yeah, I would probably fold this.  That flat call from the button looks super suspicious and doubt that the button has any intention of folding.  His range is super polarized here.  He either has a monster or he has poop because he's probably going to shove all his medium strength hands.  Since he knows that he has to show the hand down against at least one opponent it's very unlikely that he has a hand that favors your shove.

My action would change a lot depending on my perception of villain.  Absent reads or if V is strong I might just fold because I'd be worried about the flat representing strength/trapping.

How likely is V inducing you to shove?  How likely is V to check down after the flop because there's a dry side pot? In a live tournament vs a weak opponent I might flat here and hope to get to see all five cards cheaply.

Use timebank & look villain up online to see if he's a successful player, plus check the lobby to see how many tables he's playing?
