I play in a monthly League (live play).  League play is a bit different from a MTT in that there are ICM considerations from level 1 as you accumulate points based on where you finish.  (Stacks are often around 20 BB in the middle stages, and if there are 2-3 big stacks at the final table, its not uncommon for the other stacks to be under 20 BB.)

In these scenarios, the effective stack is 20BB.  I encountered all 3 middling pair situations on Friday night where I felt like I wanted to have some time with a computer before deciding.

Scenario #1: You have 88 in MP and open for 2.5BB.   Folds to the SB (very good cash player) 3 bets to 6BB.  BB folds.  We flat (because we have position).  Flop comes Q65 and he c-bets 5BB (so half his stack is in the pot).   Action?

Scenario #2: Open from MP with 77 and the Hijack (TAG, not scary player) 3 bets to 6BB.  The BB flats.   Call, Raise, Fold?

Scenario #3 : You have 66 in the SB.  A MP solid TAG opens for 2.5BB, and the Cutoff (SLAG solid) 3 bets to 6BB.  You are in the SB with 66-88 and the BB is already holding his cards to hold.   Action is to you.

How do you want to play these scenarios and why?  and what is the strategy you employ for middling pairs when effective stacks are 20BB?

I am not really familiar how to adjust my plays if there are ICM considerations from hand 1, but I assume you try to be a little tighter?? But here is how I will go about playing these hands…

Scenario #1: Calling a 3-bet from the BB with 88 – I am not doing this even when IP if I can’t formulate a plan for post-flop play. When you call his 3-bet, his SPR is 1, so you can expect a shove with a variety of hands post flop. That he is betting only 1/2 pot makes me think he has flopped the best for his hand.. If he didn’t hit his AK or something, I think he might check or shove.

Scenario #2: I think this is an auto call to fish for set or OESD.

Scenario #3: I fold as I am OOP with more action to come.

This should be in the Strategy section.

I used to be in a league like this. In mine, people would blind all the way down to ladder up. If I could build a big stack, I would open/3-bet every hand I played (i.e., never flatting). They were looking for reasons to fold.

Otherwise I wouldn’t make other general adjustments. If someone raises a lot, I would look to 3-bet shove 20 bigs with a middle-pocket-pair. You want to see who’s ignoring league rules in terms of their strategy, as well as those who overreact to it. Even my open/3-bet every hand I played strategy only worked for a season or two.

This is assuming points are linear (one more point for each spot you ladder up). How does it work for your league?
